Payment Options

It is our mission to provide you with affordable, high-quality care that fits within your budget! We realize that each patient and their families have different methods of obtaining our services – whether through HMO, PPO or private pay we can accommodate them all with the proper authorizations. Co-pays are expected at the time of service. If you are unsure of your co-pay we will gladly check for you. We accept cash, debit or credit cards for co-pays. Sorry no checks.

Other Insurance Options:

  • PPO or POS: If you have a PPO or POS option with your insurance, you may call us directly for an appointment.
  • HMO: If you have an HMO, you will need authorization specifically for Five Oaks Speech Therapy Services prior to making an appointment. This can be obtained from your primary care physician.
  • Self-Pay: Payments may also be made using cash, credit/debit cards or check.

If your insurance covers our services you will receive the benefit of reduced personal costs. Insurance policies vary, so we will review your insurance and discuss with you the terms of your policy as it applies to our services.

Please bring your insurance identification card to your appointment so that we can verify your coverage and costs, if any.

Please remember that insurance is a benefit for you and/or your child. If you change insurance during the course of our services your benefits may change and you will be financially responsible if your insurer does not pay for the provided services. Please contact your insurance company or employer and learn about your benefits. Following is information on different types of insurance accepted, we are constantly updated this list so if you do not see your insurance let us know and check back for updates.

Making an Appointment

It’s easy to make an appointment at Five Oaks – just call us at (909) 824-2899 and one of our friendly team members will happily assist you. If you schedule an appointment and something comes up, please call and reschedule as soon as possible as most authorizations for service have specific time limits for services. Failure to show will jeopardize our ability to meet the goals of you or your child and may void the authorization.


Your First Visit

We want your first visit to be the most pleasant experience. Upon arrival we will have you sign in and provide you with any State, Federal and/or insurance required paperwork. This information is for your protection as well as ours and we will assist you anyway possible. Please remember to bring your insurance identification card. After the paperwork is complete we will collect any co-pay that is due and summon your therapist.

We take pride in our waiting area and trust it is comfortable for you and/or your child. Please feel free to watch our television, read our magazines and drink some water. Of course we have toys for the kids too!

If your here for your child we will take them to one of our therapy rooms while you are in the waiting room. We are happy to have families and friends of patients observe treatment sessions as long as it does not distract the patient. Prior approval from the therapist must be given. In order to keep the integrity of the session, we ask that you do not interrupt or distract the patient during the therapy session.
